Through the forms on the site (Contact, Appointment Booking, Free Consultation) we collect: first and last name, phone number, email address and, when voluntarily provided, notes about the desired treatment. For the Free Consultation we also collect your panoramic dental X-ray, data that may fall under health-related data (special category of data under Art. 9 GDPR). If you become a patient of the clinic, we also keep your clinical record (treatments performed, allergies, medications, relevant medical conditions) for internal use by medical staff only.
The processing of personal and contact data is based on the performance of pre-contractual measures requested by the data subject (Art. 6.1.b GDPR). The processing of the panoramic X-ray, as health data, is based on the explicit consent you provide by selecting the relevant checkbox before submitting the form (Art. 9.2.a GDPR).
Data is retained for the time necessary to handle your request and, subsequently, for the time required by applicable legal obligations. You can request deletion of your data at any time by contacting us.
